Based on review, the licensee did not ensure that injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services.
Evidence: During the inspection conducted on August 29, 2017, the most recent update of the injury prevention procedures for the facility had not been reviewed annually and was dated November 2015.
Plan of Correction:
The injury prevention plan was updated at our annual training day, 9/1/17 and will be included on the agenda at EVERY annual fall training day.
