Description: Based on observation, the restroom did not have paper towels. Evidence: The first restroom closest to the classroom door did not have paper towels.

Description: Based on observation, the first aid kit was incomplete. Evidence: The first aid kit was missing 1 of 2 triangle bandages.

Plan of Correction: The triangle bandages were obtained.
