Comments:
An unannounced monitoring inspection was conducted on November 3, 2017. There were 90 children present at arrival to the facility in six classrooms with 12 staff members. Two double session classrooms were not operating on the date of the inspection. Additional staff members and volunteers were on site for administrative and support purposes. Upon arrival to the facility, a group of children were observed participating in a racing game supervised by staff. The children were observed having lunch which consisted of cube steak sliders on a whole wheat bun with lettuce and tomatoes, oven fries, mixed fruit and milk. The children were also observed listening to a story, doing music and movement, and napping. The outdoor playground equipment is not being utilized at this time. Twelve children's records were reviewed. There were no medications at the center.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, in one of twelve children's records reviewed, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician: before the child's attendance or within one month after attendance. Evidence: During the inspection conducted on November 3, 2017, there was no documentation of a physical examination in the record for Child #11 (start date 9/6/17).

Plan of Correction: The manager of the Case Manager will be contacted and the child's mother will be contacted and make arrangements to have a physical as soon as possible.

Standard #: 22VAC40-185-60-A
Description: Based on record review, in four of twelve children's records reviewed, the licensee did not ensure that each child's record was documented with all required information. Evidence: During the inspection conducted on November 3, 2017, the following information was not documented in each child's record: Child #3-A complete address for one person to contact in the event that the parent cannot be reached Child #5-Previous child day care and schools attended by the child; Name of any additional programs or schools that the child is concurrently attending and the grade or class level Child #9-The name, address, phone number of one of two required persons to contact in the event of an emergency where the parent cannot be reached Child #12-A complete address for one person to contact in the event that the parent cannot be reached

Plan of Correction: Information will be forwarded to the case managers. We will get this information documented.

Standard #: 22VAC40-185-280-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that hazardous ubstances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection conducted on November 3, 2017, the following hazardous substances, labeled keep out of reach of children and caution or warning, were observed in an unlocked cabinet in Room 114: two cans of disinfectant spray, a bottle of correction fluid and one container of disinfectant wipes

Plan of Correction: Lead teacher in that classroom will double-check the cabinet for any malfunctions. The cabinet was locked while the inspector was present.
