The licensing inspector completed an inspection in response to a self- reported concern regarding allegations of staff inappropriately handling a child and a child injury that occurred on 09/27/2017 and 10/02/2017. The Department was notified on 09/28/17 and 10/03/17. The licensing inspector was on site at the facility on 10/05/17 from approximately 10:00AM to 11:15AM. During the inspection, interviews with staff were completed, documentation was reviewed and classroom observations were conducted. Additional interviews were conducted with involved parties on 10/24/17. The preponderance of the evidence gathered during the investigation supports portions of the allegation and therefore the self-report is determined valid. See the violation notice for additional details.
Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from date of receipt. You will need to specify how the deficient practice will be or has been corrected. Just writing the word 'corrected' is not acceptable. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Standard #: 22VAC40-185-550-M Description: Based on documentation review, the facility did not ensure the written record regarding a child's injury contained the required information. Evidence: During the inspection, the licensing inspector observed the Incident/Accident report for Child #2 had a line drawn through the "Action Taken to Prevent Future Incidents". Written injury records are required to contain future action to prevent recurrence of the injury. Plan of Correction: Review of requirements for "Action Taken To Prevent Future Incidents" was completed with [staff; teachers and management]. Review included direct coaching on what "actions" would be appropriate for various incidents.
