Consultation was provided on the following areas of the Child Day Center standards as they relate to children of staff: *Standard(s) 340. A. C and F. - Staffing and Supervision; *Standard 350. A. C and E - Staff-to-children ratio requirements; *Standard 330B - Resilient surfacing for outdoor play equipment. Record keeping requirements for staff records was additionally reviewed. (Standard 70.A)
An unannounced complaint inspection was initiated on 10/06/2017 and concluded on 10/11/2017 in response to allegations of inadequate supervision and care of children. Additionally, it was alleged that compliance was not being maintained in the area of staff-to-child ratios and outdoor play equipment. Based on observation, record review and staff & parent interviews the evidence does not support the allegations therefore, the complaint is found to be not valid. During the inspection violations were found in the area of staff records and posting of the findings of the most recent inspection of the center. This inspection report was reviewed and discussed with the administrator.
Standard #: 22VAC40-185-70-A Complaint related: No Description: Based on record review and interview, the center failed to ensure that staff records shall include the following: *Name, address, verification of age requirement, job title, and date of employment; and name, address and telephone number of a person to be notified in an emergency which shall be kept at the center. *Documentation that two or more references as to character and reputation as well as competency were checked before employment. *Written information to demonstrate that the individual possesses the education, orientation training, staff development, certification, and experience required by the job position. Evidence: 1. The record for staff 1 lacked the following required information: a. Job title, date of employment, age verification and emergency contact information; b. Two reference checks; c. Documentation of orientation training, education, experience and staff development. 2. Administrative staff confirmed that this information was not on file. a. Administrative staff stated that staff 1 began employment within the past two weeks of the inspection. 3. Staff 1 was observed caring for children in the licensed program during the inspection. Plan of Correction: All staff records have been updated.
Standard #: 22VAC40-80-120-E-2 Complaint related: No Description: Based on observation, the center failed to ensure that the findings of the most recent inspection of the facility were posted on the premises of the licensed program. Evidence: 1. The most recent inspection was conducted on 09/22/2017. The findings of this inspection were not posted on the premises of the licensed program. 2. Administrative staff verified that the findings of the most recent inspection were not posted. Plan of Correction: Recent inspection report was immediately posted.
