Technical Assistance:
Discussed with Director making sure that portable heaters are kept out of reach of children.

Comments:
Today an unannounced monitoring visit was conducted with Center Director. This visit took place in the morning between 9:30am-12 there were 5 classrooms with a total of 46 children with 6 staff within the supervision guidelines. The children were observed having morning circle time, story time, making reindeers, outside play, lunch and in transition for nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children records, medications, fire drill log, and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-270-A
Description: Based on observation, the following areas of the center were found not being maintained in a clean, safe and operable condition. Evidence: In one classroom, chipped/peeling paint were found along the wall. In the two of the five classrooms, rust was found along the radiators.

Plan of Correction: Director stated that she will put in a maintenance report immediately.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to cover all unused electrical outlets accessible to children with child-resistant protective covers. Evidence: In two of the 5 classrooms observed, 2 electrical outlets were missing protective covers.

Plan of Correction: Director stated that they will covered during inspection. Director also stated she will contact the cleaning company to reinforce that they cover after use.

Standard #: 22VAC40-185-350-F
Description: Based on observation and interview with Director, the center did not have written permission permission from parent and written assessment documenting children in different classroom group. Evidence: Upon arrival, in the younger 4's classroom a teacher was observed putting the child's coat on the child to take him next door due to the classroom being out of ratio once the cook (assistant) went back upstairs. Upon arrival, in the older 4's classroom a child who was not enrolled/on the attendance sheet was in the classroom.

Plan of Correction: Director stated that she will get the letter from the parent and written assessment by next week.
