An exit meeting was completed by the licensing inspector with the center's director to review the findings of today's inspection

A Subsidy Health and Safety inspection was conducted on Monday, December 18, 2017 from approximately 2:30 pm to 5:15 pm to review supplemental health and safety requirements. During this inspection, the center's director was present and assisted the licensing inspector throughout this inspection. The census for today's inspection consisted of 12 school-aged children. The licensing inspector was notified by staff that the children are out of school for holiday break and will be participating in a variety of themes at the center. The center's theme for today was Elf day. The staff to child ratios were deemed in compliance with licensing standards. The licensing inspector completed a formal observation of the children in their classroom setting in which the children were observed participating in a 10 minute reading session and completing an elf project. The licensing inspector also completed observations of parent pick ups while at the center. The staff were observed by the licensing inspector providing age appropriate supervision and addressing the children's overall needs. The inspector interacted with staff and the children when appropriate. The classroom were equipped with age appropriate toys, and supplies. The children's snack today consisted of a cupcake, gold fish crackers, and milk. During this inspection, the required postings were reviewed and determined to be in compliance. The following information was reviewed: daily attendance, emergency supplies, menu, fire and shelter-in-place drills, liability insurance policy, and children's allergies. Medication is being administered; medications, and written authorizations were reviewed for the center's compliance. There were four children?s records and two staff's records reviewed during this inspection. There are multiple staff with current MAT certification and Daily Health Observation training on site. During this inspection, the center's classroom, bathrooms, kitchen, transportation, and playground areas were inspected.
The center's first aid kit and emergency supplies were inspected.

Last shelter-in-place drills: 04/08/17 and 11/06/17
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on review of staff record, the center did not ensure that all staff completed CPR and first aid within the required timeframe. Evidence: In Staff#2's record (date of hire 6/16/16), there was no certificate of completion for CPR and first aid found in the record. Interview with staff revealed a copy of this document had not been obtained by the center from the staff at this time.

Plan of Correction: The center will plan to obtain a copy of staff CPR/First completion within the next 72 hours.
