Comments:
A Subsidy Health and Safety inspection was conducted on 01/03/2018 to review supplemental health and safety requirements. There were 40 children, ages 5 years-13 years and 3 staff present during today's inspection. The inspector reviewed 5 children's records and 4 staff records. This program operates only when school is not in session and there was no medication being administered on the day of the inspection. The children were observed in the following activities: lunch, organized indoor large motor play, preparing to swim and during swimming. The inspector reviewed the following: the parent handbook, staff policies, emergency plans and emergency supplies. The inspector discussed the following with the staff: background check requirements, annual training hours, positioning of staff while changing for swim time, reporting injuries that require outside medical treatment, and emergency drills. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Description: Based on record review and discussion with staff, the facility failed to ensure that all staff complete the required Virginia Preservice Training within 90 days of employment or subsidy vendor approval. Evidence: Staff #1 has not completed the required Virginia Preservice Training; the documented date of hire for Staff #1 is 06/25/2015.

Plan of Correction: Staff #1 will complete the Preservice Training.
