We discussed the requirements for infant daily records and the child abuse registry background check requests from other states. The risk assessments for violations were provided. Also, we reviewed requirements for diapering procedures, information for school age children, staff education information, used linen storage in classrooms, and posted emergency procedure maps. Also, we discussed the standard requiring tummy time amounts that must be documented for each period of time young infants have active awake time on their stomachs. We discussed using cones or other marking objects to keep children away from the sections of surfacing in disrepair near the "duck" until the repair is made.
Thank you for your assistance during today's unannounced monitoring inspection conducted from 10:15 AM to 4:58 PM. Today, there were eighty-one children, ages infancy through school age, with thirteen staff. I viewed program activities, daily routines, staffing, supervision, interactions with children, nutrition/meals and snacks, infant care and records, posted information, agency inspection reports, insurance, emergency supplies and drill records, injury records, eight records for children, medication, eleven staff records, staff qualifications, staff training, equipment, indoor and outdoor areas. The children have enjoyed a variety of age appropriate seasonal activities, group discussions and access to toys, equipment and materials. The staff were observed providing frequent interactions with children and responding to individual needs. Activities with adults in the adult day care program have been a regular part of the experiences for children at the center. Let me know if you need any assistance. 540/430-9259
Standard #: 22VAC40-185-420-E-1 Description: Based on a review of the daily records for infants, and interviews with staff, there has been inconsistent documentation of the amount of time infant 1 spent on his stomach during active awake periods as required for infants who do not roll over on their own. Evidence: Of the eighteen daily records viewed for infant 1 for attendance in November, there was only documentation about efforts to provide tummy time on six dates (November 2, 10, 13, 16, 17, 29). The section for documenting tummy time was left blank on November 1, 3, 6, 7, 8, 9, 14, 15, 20, 21, 27 and 30, 2017. Plan of Correction: The center administrative staff will work with the infant staff and review the requirements for documenting the amount of time infants who do not roll over on their own have tummy time throughout the day. The infant staff will make sure all required documentation is completed at the time so that daily records available to parents and other staff provide the required communication in writing. Administrative staff will conduct periodic reviews of the daily sheets to make sure all records are maintained as required.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on a review eleven staff for determination of compliance with background checks, and interviews with administrative staff, the requirements for requesting and obtaining child abuse and neglect registry checks by other states that staff member 5 and 7 resided in the preceding five years were not adhered to. Evidence: Staff member 5 began work September 1, 2017 and indicated in writing the locations of residence in two other states in the past five years. Staff member 7 provided information in writing in August 2017 regarding residence in a state other than Virginia in the past five years. Administrative staff indicated that the process for requesting registry checks in the disclosed states were not initiated by the center in September or August for staff members 5 and 7. Plan of Correction: The center director immediately read over the instructions of requesting the child abuse and neglect registry checks in each state where staff resided and will obtain the information. All related information will be kept in the staff files and any needed communication will be conducted with the state agencies. The administrative staff will comply with requirements for obtaining child abuse and neglect registry checks when any staff disclose previous residence in other states during the previous five years at the time of employment.
