Violations:
Standard #: 22VAC40-185-280-B
Description: Based on inspection of the facility, the center did not ensure all hazardous substances were kept in a locked place. Evidence: The licensing inspector observed a container of bleach water on the shelf which opens to the kitchen. The cabinet in the kitchen contained cleaners and the unit was not locked.

Plan of Correction: Per Center: The director will make sure everything is put away and place a lock on the kitchen door.

Standard #: 22VAC40-185-330-B
Description: Based on observations and measurements, the center did not ensure outdoor climbing equipment was placed over sufficient resilient surfacing and at least six feet from a barrier or other equipment. Evidence: The climbing unit on the toddler/two's playground is three feet from the enclosure barrier and placed on resilient material that measures 1/2 to 1 inch in depth. This unit would be required to have six inches of resilient material under and around the unit.

Plan of Correction: Per Center: I will remove the climbing unit.
