An unannounced renewal inspection was conducted on February 7, 2018 during the before care portion of the program. There were 11 children present at arrival to the facility with two staff members. Throughout the course of the inspection there were 29 children present with three staff members. The children were observed arriving to the program, playing with various tabletop activities to include checkers, blocks and creative arts. The licensing inspector reviewed the physical plant, supervision, required postings, children's records, emergency evacuation and shelter-in-place documentation, and nutrition. The posted menu indicated that today's snack would consist of bagels and cream cheese. There are no medications present in the center at this time. Five children's records were reviewed during the inspection. The exit interview was conducted with the Site Director.
Standard #: 22VAC40-185-150-B Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each child's medical report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department. Evidence: During the inspection conducted on February 7, 2018, the immunizations for Child #2 had not been signed by a physician, his designee, or an official of a local health department. Plan of Correction: Will have the parents to get a signed copy of the immunizations.
Standard #: 22VAC40-185-60-A Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each child's record was maintained with all required information. Evidence: During the inspection conducted on February 7, 2018, the record for Child #2 was not documented with the following information: Name of any previous child day care and schools attended by the child; work phone number and place of employment for both parents; name, address, and phone number of one of two required persons to call in an emergency if a parent cannot be reached Plan of Correction: We will make sure that we get all information from the parent and include it in the record.
Standard #: 22VAC40-185-90--A Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that all required written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. Evidence: During the inspection conducted on February 7, 2018, in the record for Child #2, there was not a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. Plan of Correction: We will get all the information from the parent and have them to sign the appropriate paperwork.
Standard #: 22VAC40-185-240-D-5 Description: Based on observation and inspection of the facility, the licensee did not ensure that there shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation (DHO) of children. Evidence: During the inspection conducted on February 7, 2018, there were three staff members present and there was no documentation of DHO training. Plan of Correction: The Area Manager has been contacted and all staff will be scheduled to get the training.
