Comments:
A Subsidy Health and Safety inspection was conducted on 2/9/18 to review supplemental health and safety requirements. Inspection started at 10:45 a.m. and concluded at 1:30 p.m. Ten staff files reviewed. Ten children files reviewed. Classroom observations conducted. No medication on site. Conversations were held with staff and children. All classrooms were in compliance with staff-to-child ratios. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements . Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-006-B-11
Description: Based on review of children records the center did not have all required information on file. Evidence: On 2/9/18, child #1 had an allergy to seafood listed on the enrollment record, however there was no allergy plan from a physician in place.

Plan of Correction: Family started 2/5/18, mother stated that this was a food preference at enrollment and was instructed to submit documentation and allergy plan from pediatrician as soon as possible. In the mean time child has been placed on our allergy list as a precaution. Mother has been advised to submit allergy plan from doctor by 2/15/18.
