Standard #: VENDSUB-000-006-B-11 Description: Based on record review of children's files, it was determined the program did not obtain a written allergy care plan for child #4 who had a listed food allergy from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. Child #4 has a listed food allergy to scallops and the parent recorded that Benadryl should be given. 2. The Medical report for the child did not include the food allergy and the file did not contain a plan of action for the child in the event of a suspected and/or confirmed allergic reaction. Plan of Correction: We will review all files to make sure all are in compliance and will request a physician's plan of action for child #4

For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.Virginia Quality is a voluntary quality rating and improvement system for early care and education facilities serving children ages birth through pre-K.
