Today an unannounced renewal visit was conducted with Center Director. This visit took place in the morning between 10:15-1:00pm there were 3 classrooms with a total of 70 children with a total of 9 staff within the supervision guidelines. The children were observed having morning snack, circle time, counting, learning about planets, outdoor play and dismissal. Positive interactions were observed. Snack consisted of rice cakes, oranges and cheese. A complete inspection of the physical plant, children and staff records, fire drill log, medications, fire inspection, liability insurance and emergency procedures. Standard #: 22VAC40-185-510-D Description: Based on observation and review of the medication, a medication was found with an expired authorization form. Evidence: Child #1's medication was found with an expired medication authorization (12/12/17). Plan of Correction: Center Director stated that the medication form will be returned to the parent and will discuss.
Standard #: 22VAC40-185-510-N Description: Based on observation, a long-term expired medication was found on-site beyond the 14 days beyond for parent to pickup or center to discard. Evidence: Upon arrival, 2 expired epi-pens were found beyond the expiration date and the 14 days for parent to pick or to discard. Plan of Correction: Center Director stated that parent will be notified today and medication will be returned to them.
Standard #: 22VAC40-185-550-D Description: Based on observation and interview, center did not have documentation of two shelter-in-place drills per year. Evidence: no documentation of a shelter in place drill for 2017. Plan of Correction: Center Director stated that they forgot to complete the shelter in place but will do it.
Standard #: 22VAC40-185-560-G Description: Based on observation of he food containers brought from home, some containers were not clearly dated and labeled in a way that identifies the owner. Evidence: 4 lunch containers were found without a name and or date. Plan of Correction: Center Director will send a reminder to parent to make sure to label and date all container. Staff will also check daily to make sure they are all labeled.
