An exit interview was conducted with the owners of the facility to discuss the results of today's inspection. Consultation was given regarding updated background check requirements. Based on new regulations staff are not permitted to start until satisfactory background checks have returned.
An unannounced renewal inspection was conducted today, February 16, 2018 and concluded February 21, 2018 to determine the center's compliance with licensing standards. The inspection was initiated at 11:00am and concluded at approximately 2:15pm.Upon arrival, the following ratios were observed:
Two year olds: 2 staff/ 10 children
Three-Four year olds: 2 staff/ 11 children
Children were observed interacting with staff, playing with toys, using the restroom, eating lunch, and asleep on mats with the proper bedding. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All areas were inspected to include classrooms, main hallways and children's restrooms. The outdoor playground was not inspected due to inclement weather.
The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, food menu, previous inspection, and various parent information. These postings are located on the bulletin board upon entering the center and in children's areas. The center's first aid kit and emergency supplies were inspected with additional supplies found. Annual health and fire inspections were reviewed and found to be updated. Eight children's and two staff records were inspected with the majority of the required information found captured; see attached notice. The following was also inspected: MAT certifications, fire/shelter-in-place drills, playground safety plan, injury prevention plan/updates, parent agreements, and other pertinent information. Evidence: During the inspection of administrative records, the required injury prevention procedures were not available to review. Licensing standards require centers to develop written injury prevention procedures and to maintain this documentation for review. Plan of Correction: Corrected on site.
Standard #: 22VAC40-185-40-J Description: Based on the inspection of administrative records, the required annual updates were not conducted. Evidence: During the inspection of administrative records, the required annual injury prevention updates for the year 2016 and 2017 were not conducted. Licensing standards require centers to update their injury prevention procedures annually based on the the documentation of injuries and a review of the activities and services. Plan of Correction: Corrected on site.
Standard #: 22VAC40-185-580-B Description: Based on an inspection of the center's van, the first aid kit did not contain the required item. EVIDENCE: During the inspection of the center's van, it was discovered the first aid kit did not contain a set of scissors. All other required materials of the first aid kit were present. Plan of Correction: Corrected on site.
Standard #: 22VAC40-191-40-D-1-A Description: Based on the inspection of administrative records, the required documentation was not maintained. Evidence: During the inspection of administrative records, it was discovered that staff #1 did not have documentation of the criminal record check previously submitted; while staff #2 did not have documentation of the central registry and criminal record background checks previously submitted. Plan of Correction: Per provider: We previously received the background checks with a clear finding. However, we gave the reports to the previous licensing inspector and forgot to maintain a copy for our records. We will submit new reports and keep copies at all times.
