Comments:
An unannounced monitoring inspection was conducted from 10:00am-11:30am. Upon arrival there were about 155 children in care with 18 present. A sample of 2 staff and 9 children's records were reviewed. There were no medications at the time of inspection. Preschool children were observed during toileting and playing with toys in the classroom. Toddlers were observed on the playground. Administration was observed giving a parent a tour of the center.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center, inside and outside were maintained in a safe condition. Evidence: On the playground one of the plastic houses had missing connectors and the plastic around that area were cracked with sharp edges. The metal gate used to enter the preschool playground had a broken area with a protruding nail.

Plan of Correction: The director provided the following corrective action plan: One of the administrators will fix the gate and discard the playhouse.
