An unannounced monitoring inspection was conducted this date. A total of 50 children with 2 staff present at the beginning of the inspection which placed the center above the staff to child ratio allowed. Observation of the programming was made. Children were observed arriving to the program area directly after school and placing their belongings in their individual bins. Ample supplies, games, crafts and toys available for the children to freely select and participate with. Small groups of children were observed coloring with colored pencils on activity sheets provided by staff, playing legos, reading and talking with each other. Hand washing procedures observed prior to snack. Snack today was capri sun and yogurt. A total of 7 children's records were reviewed today. 6 medications on site and were reviewed. MAT (medication administration training) training and first aid/CPR certifications were checked today. Staff records are reviewed annually in the administrative office. Standard #: 22VAC40-185-350-E-5 Description: Based on observation of the program, ratios were not maintained. Evidence: When the record of attendance was completed at approximately 2:40pm, a total of 50 children with 2 staff were on site. Out of these 50 children, there was at least one 5 year old. The center remained out of ratio until a third staff person arrived at approximately 2:55pm. Plan of Correction: We have a vacancy for a third staff and are using substitutes. We will ensure that all staff are present prior to the start of the program each day.
