Comments:
An unannounced monitoring inspection was conducted today, March 20, 2018 to determine compliance with Licensing standards as a follow-up the issuance of the Conditional license. The inspection was initiated at 4:05 pm and concluded at 5:20 pm. Upon the inspector's arrival school age children had finished SOL testing earlier and were now participating in work cycles of their choice. Children were drawing, building with legos, reading and playing a globe game. The globe can consisted of children spinning the globe, pointing to a spot and telling as much information as they could about the area. All areas of the facility were inspected. The facility has age appropriate materials and supplies. Children provide their own meals and snacks. The first aid kit and non-medical emergency supplies were inspected and found complete. The following required information was posted: emergency number, the license, last inspection notice, evacuation map/procedures, and daily schedules. The inspector interacted with staff and children in care. Five staff records and five children's records were reviewed. Medication, authorizations and medication logs were reviewed (see violation notice). Staff have current CPR, First aid, daily health observation training and MAT certification. Standard #: 22VAC40-185-510-G
Description: Based on review of medications, authorizations and documentation the medication administration log, the facility did not ensure medications administered to children were maintained in the original, labeled container. Evidence: 1. Documentation stated that two medications for child #6 had been administered on 03/09/2018. 2. The two medications for child #6 did not have the original, labeled container.

Plan of Correction: Per the Director: I have e-mailed the parent to bring in printed label tomorrow.
