Comments:
This unannounced monitoring inspection was conducted on 3/20/18 from 9:30 am until 12:30 pm. At the time of the inspection 47 children were present with 10 staff members present. The sample size consisted of five staff's records, five children's records and three diaper ointments. Children and staff were observed during free indoor play, center activities, reading time singing , dancing during educational lessons, art activities, lunch, nap, hand washing, bathroom break, transitions, and behavioral management. The violations from the previous inspection were reviewed for current compliance. No repeat violations were found. The violations found during this inspection are on the violation notice. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, the director failed to keep written information to demonstrate that a new employee completed orientation training required by the job position. Evidence: 1. The record for staff 1 did not contain documentation of completed orientation training required by the job position. 2. The director stated all the required orientation was completed but was not documented in staff 1's record.

Plan of Correction: From now on all completed orientation training required by the job position will be documented and kept in each staff's record.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the administration failed to ensure all areas and equipment in the center are maintained in a safe and operable condition.

Evidence:

1. In the front toddler room paint is peeling on the window's trim.
2. There are rest mats in the back toddler classroom and pre-K room that are torn along the seam exposing the absorbent sponge.

Plan of Correction: The paint will be repaired and the rest mats will be repaired or replaced.

Standard #: 22VAC40-185-280-G
Description: Based on observation and interview, the center staff failed to label a spray bottle of hazardous substance to indicate its contents.

Evidence:

1. A spray bottle of yellow cleaner is in the infant room and it does not have a label.
2. The staff stated the spray bottle contained a disinfectant for diapering.

Plan of Correction: The administrator will make sure the bottle is labeled and will talk to staff about checking to make sure the label does not wipe off the bottle.

Standard #: 63.2-1720.1-A
Description: Based on record review, interview and observation, the director failed to ensure a completed fingerprint background check was obtained prior to employment for a staff member hired after 1/22/18. Evidence: 1. Staff 1 was observed working in one of the downstairs classrooms. 2. The record for staff 1 shows a start date of 3/14/18. The record did not contain documentation of a completed fingerprint background check. 3. The director stated she did not have a fingerprint background check for staff 1.

Plan of Correction: Staff 1 was scheduled on the day of the inspection to obtain fingerprints on 3/23/18 and they were returned as eligible for employment on 3/23/18 and forwarded to the inspector. In the future no staff will begin employment until fingerprint background check results have been received as eligible to work.
