- Technical Assistance:
Technical assistance today included record keeping, physical environment and incident reports.
An unannounced monitoring inspection took place today from 11:00 am - 2:20 pm. There were 68 children present with 11 staff. I observed children listening to a story, coloring, playing with transportation toys and preparing for school photographs today. I also observed preparation for lunch and naptime. Records were well-organized and classrooms were filled with opportunities for play. If you have any questions, please contact me at email@example.com
Standard #: 22VAC40-185-130-B Description: Based on a review of five children's files today, one child did not have documentation of additional immunizations as required. Evidence: Child # 2, (dob 5/16, enrollment 8/16), had records of immunizations at 2 months but not every six months as required for children under age two.. Plan of Correction: I will talk to the mother today.
Standard #: 22VAC40-185-270-A Description: During today's inspection, some areas and equipment of the center were observed to be not maintained in a safe and operable condition. Evidence: Indoors: The cabinet door to a diaper table was not hanging securely. The baseboard moulding surrounding a pole was loose in one classroom, posing a tripping hazard. Outdoors: Staff report that aluminum fence was knocked over go during a storm, but it is still down on one side and a long loose pipe is accessible from the fence. Plan of Correction: We will place a work order today for the indoor work. A tree had fallen on the fence during the storm and that work was completd but we will contact them to come out and finish the work. Until then, that are will be off-limits.
Standard #: 22VAC40-185-430-B Description: Based on observation today, there was a toy in one room that was not age and stage appropriate. Evidence: In the infant room today, there was a wide selection of age appropriate stuffed animals, but an infant was observed playing with a stuffed rabbit that had a plastic nose. The infant was observed mouthing the plastic nose. Plan of Correction: Removed during inspection. I will talk to the teachers.
Standard #: 22VAC40-185-510-C Description: Based on a review of children's medication today, one child's medications no longer had a current authorization date. Evidence: Two long-term allergy medications were present for Child #1 today, but the authorization for for one medication expired 8/17. Plan of Correction: I will contact the parent today.
Standard #: 22VAC40-185-520-C Description: Based on observation during today's inspection, one classroom had diaper creams accessible at child height. Evidence: Although diaper creams do not need to be locked, they are required to be inaccessible. They were observed on the bottom of an unlocked cabinet under the diaper table in the two year old room. Plan of Correction: Corrected during inspection. I will talk to the staff about this.
Standard #: 22VAC40-185-550-M Description: Based on a review of incident reports in March, some items were missing required information. Evidence: Of six files reviewed, 6 had no information regarding the time and method the parent was notified of the incident/injury; and 3 had only one signature. (two are required.) Plan of Correction: We will be going over the incident reports in detail.
