Comments:
A complaint investigation was initiated on March 13, 2018 regarding falsifying documents in staff records, mold in the pre-kindergarten classroom, and spoiled food being served. Interviews were conducted, observations completed, and documentation and staff records were reviewed. While all areas of the complaint were investigated, areas of non-compliance with licensing standards that have been determined as valid are identified in the Violation Notice.

Standard #: 22VAC40-185-70-A
Complaint related: Yes
Description: Based on review of six staff records, the center did not maintain complete staff records. Evidence: The records for staff #1, #2, #3, and #4 did not have information to demonstrate that the individuals possessed the education and experience required by the job position of lead teacher.

Plan of Correction: All staff files have been reorganized. Lead qualifications are now placed in front of folders for easy identification.

Standard #: 22VAC40-191-40-D-1-C
Complaint related: Yes
Description: Based on review of six staff records, the center did not update criminal history record reports every three years with original copies, dates, and signatures. Evidence: The record for staff #1 (start date: October 10, 2013) was not an original update criminal history report with results. The record was a photocopy with a white-out substance over the original date. The date October 13, 2017, was handwritten over the white-out substance.

Plan of Correction: Everyone at center has been trained on the importance of maintaining original documents in files. All files have been audited to ensure all records are accurate and up to date, as well as all fingerprints have been run and returned for current staff.

Standard #: 22VAC40-191-60-C-1
Complaint related: Yes
Description: Based on review of six staff records, the center did not have an original criminal history record report within 30 days of employment of a staff member. Evidence: The record for staff #1 (start date: October 10, 2013) did not have an original criminal history record report.

Plan of Correction: Everyone at center has been trained on the importance of maintaining original documents in files. All files have been audited to ensure all records are accurate and up to date, as well as all fingerprints have been run and returned for current staff.

Standard #: 22VAC40-191-60-C-2
Complaint related: Yes
Description: Based on review of six staff records, the center did not have an original central registry finding within 30 days of employment of a staff member. Evidence: The record for staff #4 (start date: August 22, 2016) did not have an original central registry finding completed until February 6, 2017.

Plan of Correction: Consistency with monitoring new staff records making sure they are complete within 30 days of required compliance.
