Technical Assistance:
Background check requirements were reviewed. MAT training information will be sent to the director. The new application was reviewed.

Comments:
A Subsidy Health and safety inspection was conducted on 4/5/2018 to review supplemental health and safety requirements. The inspection began at 3:45 p.m. and ended at 5:45 p.m. There were 27 children in care with 3 staff and one intern supervising at all times. The children were observed playing outside, working on homework, being picked up by parents, reading, doing a craft, and playing with balls in the gym. Five children's records were reviewed. One parent was interviewed. One new staff person's record was reviewed at the main office from 1:30 a.m. until 2:15. A licensing inspection was conducted on the same date as the SHSSI inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Area of non-compliance are identified on the violation notice. Description: Based on record review and interview, there was not a written allergy plan for each child with a diagnosed food allergy. Evidence: 1. The inspector reviewed one child's record who had medication on hand for an allergy and there was information in the child's file about the allergy but there was no allergy plan. 2. The director also looked for the allergy plan and could not find it.

Plan of Correction: The director will ask the school nurse if there is already an allergy plan and if not the director will get one from the parent.
