Technical Assistance:
An exit interview was conducted with the director to discuss the results of today's inspection.

Comments:
An unannounced monitoring inspection was conducted today, April 9, 2018 to determine the center's compliance with licensing standards. The inspection was initiated at 2:40pm and concluded at approximately 5:00pm. Upon arrival, the following ratios were observed:

Two-Three year olds: 4 children/1 staff
Four-Five year olds: 8 children/1 staff
School age room #1: 7 children/ 1 staff
School age room #2: 11 children/1 staff

Children were observed playing with toys, reading, coloring pictures, and interacting with staff. All areas used by children were inspected to include the main hallways, children's restrooms, classrooms, and school age building. Due to previous rain the playground was not inspected.

The center had the required postings displayed on the wall upon entering the building. The center's first aid kit and emergency supplies were inspected with the required supplies found. Ten children's records and three staff records were inspected with the majority of the required information present. The following was also inspected on this day: annual fire & health inspections, fire/shelter-in-place drills, playground safety plan, daily attendance log, parent agreements, and other pertinent information.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on the inspection of administrative records, the required information was not updated. Evidence: During the inspection of three staff records, it was discovered staff #2 had a TB (tuberculosis) test in the record dated 11/4/15. This TB test expired on 11/4/17 and an updated TB test or screening should have been obtained prior to expiration.

Plan of Correction: Per director: I will have the staff to get an updated TB test and put the results in the record.

Standard #: 22VAC40-185-60-A
Description: Based on the inspection of administrative records, the required information was not listed. Evidence: During the inspection of ten children's records, it was discovered child #5 did not have the physical address listed for the second emergency contact.

Plan of Correction: Per director: I will obtain the information from the parent tomorrow.

Standard #: 22VAC40-185-240-D-5
Description: Based on the inspection of administrative records, the required training has not been obtained.

Evidence:
During the inspection of staff records, it was discovered there were no staff present today with daily health observation (DHO) training. Licensing standards stipulate there shall be at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

Plan of Correction: Per director: I will have the training coordinated so all staff are trained in daily health observation.

Standard #: 22VAC40-185-270-A
Description: Based on the inspection of the inside of the building, some areas were found in an unsafe condition. Evidence: During the inspection of the building, chipped paint was found on the baseboard of the children's orange room and yellow room.

Plan of Correction: Per director: I will talk with the owner and schedule for the rooms to be painted.
