63.2 Facilities and Programs..

Technical Assistance:
Due to the serious nature of the violations, the Inspector will return to review that ratio and medication procedures have been corrected. Ratio is based on the youngest child assigned and present in the group. Fire Evaction Drills are required to be completed every 30 days and Shelter-in-place drills are only required twice a year.

Comments:
An unannounced inspection was conducted at 9 AM today. The Inspector arrived to find 6 children ( 2- 4years) with 2 staff and the Director was absent today. The Inspector observed a younger child in one group and found there were 3 children two years off age enrolled and regularly present in the group. The Inspector also observed the emergency medication stored at the center for a child expired a year ago. The Inspector also reviewed records pertaining to accident reports and medication administration. The Inspector did not review staff records because a Director was not present and a staff member could not leave the room to get files. The Inspector will return to check records. Standard #: 22VAC40-185-290-3
Description: Based on observation of older Preschool room, the center did not have an outlet covered. Evidence: 1. The electrical outlet by the white bulletin board was missing one cover.

Standard #: 22VAC40-185-350-C
Description: Based on record review and staff interviews, the center has not kept staff-to-child ratio in compliance and applicable to the youngest child in the group. Evidence: 1. Young preschool room has 3 children who are two years of age. The attendance sheet shows there are 12 children enrolled in the class and only one staff for the class. The Inspector counted 7 days in March and 6 days in April where there were more than 8 children present in the class when a two year old was also present. 2. At 957 AM, the young preschool had 9 children with only one staff and there were two children present that were two years of age. 2. Ratio for the two year old children is 1 to 8.

Standard #: 22VAC40-185-510-A
Description: Based on record review, medication check, and staff interviews, the center is not following the center's written medication policies. Evidence: 1. Child #1 has a written food allergy and paperwork states child would require an Epipen in the event of an allergic reaction. 2. Staff handed the Inspector the medication and the medication does not contain the prescription label. Upon further review of the medication, the Inspector observed the expiration date is February, 2017. 3. The long term written authorization for the medication was found and it only contains the parent's written authorization and does not include the physicians authorization.
