Discussed requirements for children's files. Suggested a tickler checklist to ensure that documents are obtained timely.
An unannounced monitoring inspection was conducted this date. A total of 92 children with 15 staff were present during the inspection. Walk thru of all classrooms was conducted. Observation of diapering, hand washing and transitions to nap time was made. The center was decorated with painted artwork completed by the children, goldfish cracker graphing, the letter Z activities and geometric shapes completed by children using marshmallows and pasta. The infant board was current regarding feeding and diapering times. A total of 9 children's records and 5 staff records were reviewed today. Staff training and topical ointments will be reviewed at the next licensing inspection and were not reviewed this date. This inspection began at 12pm and ended at 3:10pm. Standard #: 22VAC40-185-130-A Description: Based on review of 9 children's records, 3 out of the 9 records did not have documentation that immunizations were obtained prior to enrollment. Evidence: Child #5, # 7 and #3 began care before immunizations were obtained for each child. Plan of Correction: We will audit all files and ensure that going forward immunization documentation is obtained prior to start date.
Standard #: 22VAC40-185-160-A Description: Based on staff record review, one TB screening was not obtained as required. Evidence: Staff # 1 began employment on 4/16/18. The TB screening was dated more than one year prior to the start of employment. Plan of Correction: We will ensure that all TB screenings are no older than one year from their employment date.
Standard #: 22VAC40-185-60-A Description: Based on child record review, some records were found incomplete. Evidence: Child #6's record did not contain addresses for both emergency contacts. Plan of Correction: We will audit all files and ensure that addresses for two emergency contacts are obtained for each child.
Standard #: 22VAC40-185-350-F Description: Based on staff statements and observation of the Pre K classroom, one child was temporarily assigned to another classroom in a casual manner, disrupting the child's schedule and attachment to the staff and group. Evidence: Child #8 was transferred to another classroom during nap time for the purpose of maintaining ratios in the Pre K classroom. Plan of Correction: We will no longer move this child or any other child to another classroom to maintain ratios at any time.
