Standard #: 22VAC40-185-140-A
Description: Based on record review, each child did not have a physical examination on record within 30 days of enrollment. Evidence: 1. The inspector reviewed five children's records. One of five records (#4) did not have physical examination in the file within 30 days of enrollment.

Plan of Correction: The director will get the physical for this child and make sure other files are complete.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center did not have all the required information in the children's files. Evidence: 1. The inspector reviewed five children's records. Three of five records did not contain all the required information. File #1 had no information about allergies, file #3 had no info. about action to take in case of an allergic reaction to an allergen, file #5 had no action to take for a diagnosed food allergy listed. Child record #4 and #5 did not have proof of birth.

Plan of Correction: The director will get the required information from the parents.

Standard #: 22VAC40-185-500-A
Description: Based on observation, center staff did not wash hands after diapering. Evidence: 1. The inspector observed a staff person changing a diaper and did not wash their hands afterwards.

Plan of Correction: Director will review hand washing requirements.

Standard #: 22VAC40-185-550-M
Description: Based on record review, the center did not complete the injury report forms with all the required information. Evidence: 1. The inspector reviewed injury reports and two did not have action to prevent future recurrence.

Plan of Correction: The director will review with the staff the forms, forms will be reviewed for completeness.

Standard #: 22VAC40-185-570-E
Description: The center did not date and label all prepared infant formula. Evidence: 1. The inspector observed a unlabeled bottle in the refrigerator. Staff stated the parent had brought it in.

Plan of Correction: Staff will label all bottles with the date and name. Bottle was disposed of during the inspection.
