Technical Assistance:
LI discussed generic epinephrine auto injector with staff.

Comments:
An unannounced monitoring inspection was conducted today from approximately 12pm-1pm. There were 14 children in care directly supervised by 2 staff. A sample of 5 children?s records, and 2 medications were reviewed. Taking a nap. Zero new staff have been have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the staff. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov.

Violations:
Standard #: 22VAC40-185-40-H
Description: Based on observation, the facility did not maintain public liability insurance. Evidence: The liability insurance plan expired September 1st 2017.

Plan of Correction: We will get the new plan from the office.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: 1) A solution labeled "bleach and water" was found on the window ledge in the kitchen. 2) Dish washing liquid labeled "keep out of reach of child" was found on the counter in the kitchen. 3) Foaming sanitizer labeled " keep out of reach of children" was found on top of the grey changing table in the bathroom.

Plan of Correction: The foaming sanitizer and the "bleach" solution belong to the school we share the bathroom and kitchen with them. The dish detergent will be locked in a cabinet. The school will have their own lock box for their solutions.
