An unannounced monitoring inspection was conducted today 4/23/2018. The inspector arrived at 9 :30 A.M. and departed the program at 1:30 P.M. There were 110 children present with twenty direct care staff members. During the inspection all eleven classrooms were observed, lunch time was observed, infant care was observed, diapering was observed, hand washing was observed, nap time was observed, emergency supplies were viewed, six medications along with authorizations were reviewed, policies and procedures were reviewed, and a sample of injury reports were reviewed.
Standard #: 22VAC40-185-340-G Description: Based on observation, the center failed to ensure that all sleeping infants were within actual sight and sound supervision of staff. Evidence: 1. Observed infant # 11 in the napping room while one staff was labeling bottles by the sink and the other staff person was sitting in the rocking chair feeding another infant. There is a window that would allow the staff member to see the infants in the napping room. The infant was not able to be heard due to the music that was playing in the napping room. The staff person that was labeling the bottles was facing the opposite direction of the window when the inspector entered the classroom and was unable to see the infant. The infant was out of sight and sound supervision. Plan of Correction: The PD will remind the infant teachers that all infants must be within sight and sound supervision at all times.
Standard #: 22VAC40-185-510-A Description: Based on review of medications, the center failed to ensure that all medications were not expired. Evidence: 1. Medications for child # 12 was expired on 1/18. Plan of Correction: The parent will be notified to pick up the expired medication. Administration will put together a medication sheet that will list the medications and the expirations dates and permissions and usages to maintain future compliance.
