22VAC40-191 Background Checks (22VAC40-191)
An unannounced inspection was conducted from 945- 1215 today. The Inspector walked through classrooms, interviewed staff, and observed programs in the two's class, PreK, and infant class. Hand washing, diapering, sanitizing, and circle time was observed. Files, training documents, and attendance were reviewed. Please call with questions 703-479-4679
Standard #: 22VAC40-185-70-A Description: Based on record review, staff # 2 does not have complete documentation of references. Evidence: 1. References do not include the date each one was conducted. Plan of Correction: Sign and date the references.
Standard #: 22VAC40-185-270-A Description: Based on observation, there was equipment that was not maintained in a clean manner and the heat covers are not maintained. Evidence: 1. There were 3 heater covers with signs of chipped and or peeling paint. 2. The crib ends in the Infant 1's room show signs of residual disinfectant spray. The spray film was stuck on the glass and the inspector was able to scrape the substance with her nails. Plan of Correction: We put in a work order for the painting. We will remind the staff to make sure that the glass is also cleaned off each day after wiping down the crib mattresses.
Standard #: 22VAC40-191-40-D-1-B Description: Based on record review and staff interviews, the center did not obtain a Search of central registry before 30 days of employment at the facility ended. Evidence: 1. Staff # 3 began employment on September 5, 2017. There was not a CPS form in the file. Management handed the form to the Inspector showing it was filled out yesterday and would be sent today. Plan of Correction: We will have the system to ensure everything is attached before they start employment.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review and interviews, an employee was not denied continued employment when a central registry finding was not obtained within 30 days of employment. Evidence: 1. Staff # 3 was hired on September 5, 2017. Staff was present today and observed during the inspection. 2. Records reviewed and staff interviews revealed the CPS check for staff #3 has not been sent. Plan of Correction: We are sending her home today.
