Comments:
An unannounced inspection was conducted on 04/12/2018 from 9:40am - 2:45pm in response to a self-reported incident which was reported on 04/11/18 regarding supervision of children. Based upon interviews, name-to-face logs, and records reviewed it was determined that the preponderance of evidence does support a violation, which can be found on the violation notice. Violations:
Standard #: 22VAC40-185-340-F
Description: Based on interviews, the center did not keep a child under the age of 10 within sight and sound supervision at all times. Evidence: The Licensing Inspector interviewed staff#1 who was on the adjacent playground with the preschool classroom at the time of the incident. Staff#1 verified that while on the preschool playground, the staff member looked over the half fence to the playground used by the two's class, and noticed child#1 walking around the locked playground unsupervised by staff. Staff#1 reported that they immediately walked over to child#1, and brought the child from the locked playground into the Director's office at approximately 11:19am. Staff#1 added that the child was not crying, and did not appear to be injured. The LI also interviewed staff#2, and staff#3, they both indicated that child#1, age two, and the entire class, proceeded to leave the playground, located right outside the classroom, to enter the classroom at 11:15am. Staff#2 documented the name-to-face log as each child walked back into the classroom, including child#1. Staff#3 remembers talking to child#1 once the child walked into the classroom. Staff#2 stated that child#1 may have opened the classroom's door and walked back out to the playground. Staff#3 stated that it's possible that the child went back outside while the children were still coming into the classroom. Based on the timeline of accounts from Staff#1, #2, and #3, it is estimated that approximately 3 to 4 minutes elapsed between the time the child left the classroom and the time the child was found on the playground unsupervised.

Plan of Correction: 1. Following the incident we held a staff meeting on 4/11/2018 and re-trained on KinderCare's "Child Supervision Records", logs and proper supervision. 2. We immediately had the playground door in the two's inspected by maintenance. A door part is on order to correct the "swing back" of the door when it shuts.
