Comments:
An unannounced full renewal inspection was conducted today. There were 66 children in care, with a total of 5 staff members assigned to their direct supervision. Children's records, staff training and emergency records, children's nutrition, emergency preparedness items, the physical plant, and children's medications were inspected today. The children were observed upon arrival to the program, practicing for their upcoming Disney play, and preparing for various afternoon activities to include making cards for Mother's Day and Teacher Appreciation Day. The children were to be served celery sticks, cheese sticks, and water for snack later today. This inspection was completed between approximately 3:00pm and 4:30pm. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: Child records #2 and #6 do not include the information for two emergency contact people who are not the child's parents.

Plan of Correction: I will ask each parent for new emergency contacts.

Standard #: 22VAC40-185-280-G
Description: Based on observations, it was determined that one substitute container of a potentially hazardous substance was not labeled as required. Evidence: One spray bottle of cleaning solution was not labeled to indicate the contents.

Plan of Correction: The bottle was immediately labeled during the inspection.

Standard #: 22VAC40-185-510-A
Description: Based on a review of children's medications, it was determined that one authorization form has not been fully completed as required. Evidence: The authorization form for one child's long term medication authorization does not include the parent's signature.

Plan of Correction: I will ask the parent to sign the form when the child returns in the morning.
