The licensing inspector conducted an unannounced monitoring inspection from 10:15am - 2:00pm. There were 92 children in care with 18 staff. The children were observed playing in the centers, at story time, playing on the playground, during lunch and during nap time. Nine staff's records and nine children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-330-B Description: Based on observation and measurement, the child day center failed to have 6 inches of resilient surfacing under and surrounding equipment on the playgrounds. Evidence: 1. The inspector observed the preschool playground. There was approximately 3 to 4 inches of mulch at the end of the slides and surrounding the climbing dinosaurs. The fall height of the equipment is 24 inches. The inspector observed the school age playground. There was approximately 2 to 3 inches of mulch at the end of the slides, under the monkey bars and surrounding the fireman's pole. The fall height of the equipment is 48 inches. Plan of Correction: The director said mulch has been ordered and will be placed in these areas on the playgrounds by May 11, 2018. Staff will monitor the playgrounds for needed maintenance.
