Comments:
An unannounced monitoring inspection took place on February 11, 2020. There were 47 children in care from ages 6 months old to 4 years old with 8 staff providing care and supervision. The inspector reviewed five children's records and five staff records onsite. The inspector assessed two medications onsite. Children were observed during the following activities: arrival, breakfast, free time, story time, and nap. The inspector also reviewed emergency drill documentation, first aid kits, the physical plant, and emergency maps. There were discussions about medications and chemical storage. The inspection took place from approximately 8:15 AM to 1:30 PM. Violations were discussed at the conclusion of the inspection. Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children.

Evidence:

1. A spray bottle solution of soap and water along with a spray bottle solution of bleach and water were unlocked on a diapering surface in the "Bears Room." No children were in this part of the room or being diapered at that time.
2. An aerosol can of room deodorizer was observed in a bathroom connected to the infant room. The door to the bathroom was ajar and the bottle was located on the toilet commode.

Plan of Correction: Staff will make sure that all these items are locked.

Standard #: 22VAC40-185-500-B
Description: Based on water temperature taken with a food grade thermometer, the center failed to ensure that the diapering area shall be provided with a sink with running warm water not over 120 degrees.

Evidence:

The hot water running in the sink located in the "Bears Room" was hot to the touch. The inspector measured the temperature of the hot water using a food grade thermometer and the water temperature measured at 143.5 degrees.

Plan of Correction: Staff will have maintenance correct this issue.

Standard #: 22VAC40-185-510-J
Description: Based on observation and discussion with staff, the center failed to ensure that all medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:

1. In the "Tiger Room", a topical prescribed ointment for child A was located in a cabinet with other non-prescribed diaper creams/ointments. The cabinet was secured with a child-safe latch but the cabinet was not locked and was at floor level.
2. Staff in the "Tiger Room" stated that "prescriptions were not supposed to be in that cabinet."

Plan of Correction: Staff will ensure that all prescriptions will be locked.

Standard #: 22VAC40-185-550-D
Description: Based on drill record review and discussion with staff, the center failed to implement a monthly practice evacuation drill and a a minimum of two shelter-in-place practice drills per year for the most likely to occur scenarios.

Evidence:

1. The inspector found entries for monthly evacuation drills for every month except January 2020.
2. Staff verified that the January 2020 evacuation drill was not recorded.

Plan of Correction: Staff will ensure all drills are documented.
