Comments:

An unannounced monitoring inspection was conducted May 9, 2024 approximately 10:30 am to 11:25 am. There were 25 children directly supervised by 7 staff. The physical plant, 5 staff records, 2 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating snack. There was an assortment of age appropriate materials for the children in care. Description:	Repeat Violation Systemic Deficiency Based on staff records review, 3 out of 5 records, the center did not obtain documentation of a negative tuberculosis screening for each staff member and individual at the time of employment and prior to coming into contact with children.
