An unannounced renewal inspection was completed on 11/13/2019 from 2:25 P.M.-5:15 P.M. At the time of the inspection there were five children present with an additional three children arriving after school, and two caregivers present. There was a total of sixteen points. Five children records, one provider record, one household/caregiver member record, emergency plan, first aid kit and emergency supplies, evacuation drills, and annual training hours were reviewed. The children were observed during naptime, school age children arriving afterschool, handwashing, snack time, television time, free choice playtime, and departure. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-111-60-B Description: Based on review of children files, the provider failed to ensure that all required information was included in children records.
Evidence:
1. Child #1's file did not contain the insurance policy number.
2. Child #2's file did not contain the name of the child's physician, address, and phone number. The record had two emergency contacts listed, but there were no addresses listed for the emergency contacts.
3. Child #3's file did not contain the insurance policy number, and employment address and phone number for the parents of the child. Child #3's file was also missing an phone number for two of the emergency contacts, and the address for one of the emergency contacts.
4. Child #2 and Child #3's file did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located.
5. The provider verified that three out of the five children records viewed did not contain all required information.Plan of Correction: The provider will contact all parent and request all missing information.
