Thank you for your assistance during this unannounced inspection. This on-site visit was conducted on September 25, 2019 as a result of a complaint that was received by the Valley Licensing Office on September 18, 2019 regarding an incident of alleged abut that occurred at the center on March 30. 2017. Documentation was reviewed and one staff was interviewed. The information gathered during this investigation does not support the allegation. Therefore the complaint is unfounded.
