Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on September 6, 2019 as a result of a self-reported incident to the Valley Licensing Office on July 19, 2019. Interviews were conducted with three staff, one child's record was reviewed, and video footage was reviewed. Areas of non-compliance may be found on the violation page.

Violations:
Standard #: 22VAC40-185-340-A
Description: Based on review of video footage, the center failed to ensure the care, protection and guidance of one child.

Evidence:

1. At approximately 10:31 a.m. and 10:33 a.m., Staff #1 was sitting on the floor with two other children near her.
2. Child A was observed at 10:31 a.m. and 10:33 a.m. crawling toward Staff #1.
3. Staff #1 was observed spraying Child A in the face with a water bottle.
4. Child A was observed turning his head away and grabbing his left ear.
5. At approximately 10:31 a.m. and 10:33 a.m., Staff #2 was observed sitting in the rocking chair and witnessed Staff #1 spray Child A with the water bottle.
6. Staff #2 did not report the incident to administration because she did not think it was ?that bad.?

Plan of Correction: ? Staff Member 1 and Staff Member 2 were terminated from Creative Wonders Learning Center.
? All staff members were to re-take the Child Abuse and Neglect training provided by Virginia Department of Social Services.
? Staff members were also required to write a full page paper on Mandated Reporting to show they understand what it is to be a mandated reporter.
? Management typed up a paper informing staff on what it is to be a mandated reporter, some possible things to look for, and the steps to reporting abuse/neglect. Management now includes an in-depth discussion with this paper in our orientation of new-hires.
? Creative Wonders Learning Center has been a partner with The Virginia Quality Rating Center and we reached out to them to help develop new techniques for classroom management, behavior management, and self-reflection. With this, the staff had mandatory training to implement these new techniques.
? Management has updated our Policies and Procedures including the steps to reporting, staff handling their emotions and recognizing their ?breaking point?, and explaining the Open Door policy and what to do if another staff member has questionable actions.
? Management has also implemented hourly room checks to try and have a bigger presence in the classrooms and to touch base with the teachers on their emotional and physical states.

Description: Based on review of video footage, one staff roughly handled a child.

Evidence:

1. Video recording was reviewed for July 17, 2019.
2. At approximately 10:22 a.m. on July 17, 2019, Staff #1 was observed sitting in a rocking chair writing in a notebook in the infant classroom.
3. Child #1 crawled over to Staff #1 and touched her leg to pull himself up.
4. Child A grabbed Staff #1?s notebook.
5. Staff #1 was observed kicking the child with her foot.
6. Child A was observed falling backwards to a sitting position on the floor.
7. At approximately 10:25 a.m., Child A was observed sitting on the floor by the rocking chair of Staff #1.
8. Child A was observed pulling himself up towards Staff #1 and tried to grab the paper she was writing on.
9. Staff #1 was observed pushing Child A down with her hand.
10. Child A was observed falling backwards to a sitting position on the floor.
11. At approximately 11:06 a.m., children were observed sitting at the table for lunch.
12. Staff #1 was observed picking up Child A and dropping him into his seat at the table
