Comments:
An unannounced monitoring inspection was conducted on 5/14/24 from 9:30 a.m. to 10:45 a.m. with the Director. There were 24 children in care, ranging in age from three to five-years-old, supervised by three staff. The children were observed in Library and Spanish class. Four child records and three staff records were reviewed. The Center has three staff with current certification in CPR and First Aid. Two medications and authorization forms were reviewed, and the center has three staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22.1-289.035-B-2
Description: Based on review of three staff records, the center did not obtain documentation of the results of a national fingerprint background check prior to date of hire.

Evidence: The fingerprint results on record for Staff #3, with an employment date of 9/19/22, were dated 9/29/22.

Plan of Correction: I acknowledge that the fingerprint background check results should be obtained prior to the employee start date. Moving forward this process will be followed.

Standard #: 8VAC20-770-60-C-2
Description: Based on review five staff records; the center did not obtain documentation of the results of a central registry search within 30 days of employment for all staff members.

Evidence: The record for Staff #3 with an employment date of 9/19/22, the results of the Central registry search background check was dated 11/3/22.

Plan of Correction: I acknowledge that the central registry search results should be obtained prior to employee start date or mailed by day seven of the start date and due within 30 days of employment. Moving forward this process will be followed.

Standard #: 8VAC20-780-160-A
Description: Based on a review of three staff files, the center did not obtain documentation of a tuberculosis (TB) screening for staff at the time of employment and prior to coming in contact with children.

Evidence: The record for Staff # 3, with an employment date of 9/19/22, contained documentation of a tuberculosis screening dated 9/26/22.

Plan of Correction: I acknowledge that the tuberculosis (TB) results should be obtained prior to the employee's start date and before coming in contact with children. Moving forward this process will be followed.

Standard #: 8VAC20-780-330-B
Description: Repeat Violation

Based on observation, the center did not ensure that resilient surfacing complies with the minimum safety standard where playground equipment is provided.

Evidence: There was less than one inch of mulch observed at the base of the playground structures and four swings. Six inches of resilient surfacing is required at the base of slides, fall zones, and six feet on either side of the swings.

Plan of Correction: New mulch has been purchased. The maintenance Technician will rake and lay new mulch to meet the required 6 inches at the base of all playground structures and swings.
