Comments:
An unannounced renewal inspection was initiated on May 13, 2024 and concluded on May 15, 2024. Four staff were present supervising fourteen children. An additional staff was on site for administrative duties. Children were observed playing outside and eating a snack. Licensing Inspector reviewed standards related to supervision, activities, equipment, nutrition, diapering, emergency supplies, inspection reports and required postings. Five staff and five child records were reviewed. Violation found is noted on the violation notice.

Standard #: 8VAC20-780-330-B
Description: Based on review, facility did not ensure there is adequate fall zone of required resilient surfacing under and around climbing equipment or equipment with moving parts on the outside playground.

Evidence: A plastic little titles playground piece with two slides was within 1' of a child picnic table at one slide chute and within 4' of the other slide chute and a red choo-choo piece of equipment. There should be at least 6' of fall zone in these areas.
