An unannounced inspection was conducted on 01/04/2024 in response to a complaint concerning disease prevention, ratio, supervision of children, and injury reporting. Interviews were conducted, observations were made in the center, and documentation was reviewed. The information gathered support violations of the Standards for Licensed Child Day Centers. The complaint is determined to be valid. Please sign the violation notice and complete the ?plan of correction? and ?date to be corrected? for violation cited on the violation notice and return it to me within 5 business days. The plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Injury report for C1 dated 12/12/2023 documented the child while being assisted with putting on his coat complained that his arm hurt, and he was unable to raise his arm. Evidence:
Based on observation the center did not maintain the ratio of 1 staff to 8 children in the Deer Room(2 year old room). The licensing inspector observed the classroom teacher accept supervision of a 9th child at approximately 10:10 a.m. while the class was on the playground.

Evidence:
Licensing inspector observed a staff member in the Bunnies Room change a child?s diaper. The staff cleaned the diapering surface; however, failed to allow the disinfectant solution sprayed on the diapering surface to air dry for a minimum of two minutes.
