Comments:
An unannounced monitoring inspection was conducted on Tuesday, December 10, 2019 to determine the center?s compliance with licensing standards. The inspector arrived at 9:30am and concluded the inspection at approximately 12:10pm. The center?s head of school and communications director were present and assisted the inspector throughout the inspection. The census for today?s inspection consisted of 68 children in the direct care of 10 staff members. Additional staff were on site and available to the children in care. Upon arrival, the children were engaged in work centers. Each classroom was inspected and formal observations were completed. The center is equipped with age and stage appropriate materials and equipment for the children?s use. Staff were engaged with the children and were observed offering guidance when needed. All areas where the children receive care were inspected and found to be in compliance. The center had the following posted: license, daily schedules, emergency numbers, evacuation maps, menu, and various parent information. The center?s first aid kit and non-emergency supplies were inspected and found complete. During the inspection, six children?s records and five staff records were reviewed. Medication is administered and three medication were reviewed today. The most recent evacuation drill was documented on 12/10/2019. A shelter-in-place drill was documented on 02/04/19 and one more has been scheduled for December 2019.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of five staff records on 12/10/2019, the center did not obtain documentation of a negative tuberculosis (TB) screening for one staff within the required time frame.

Evidence: 1) The TB results in the record for Staff #5, re-hired on 8/19/19, were dated 8/16/17 and expired on 8/16/19. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering.

Plan of Correction: The staff will check dates of the last TB screening and will obtain a new one if necessary on 12/10/19.

Standard #: 22VAC40-191-60-B
Description: Based on a review of five staff records on 12/10/19, the center did not obtain a completed sworn statement or affirmation for one staff prior to employment.

Evidence: 1) Staff #5, with an original hire date of 10/01/15, terminated employment on 8/6/18 and returned as a re-hire on 8/19/19. 2) The licensing inspector observed a completed sworn statement in the record for Staff #5 that was dated 8/09/17. The staff had been terminated for more than 12 months and required a new sworn disclosure or affirmation prior to employment.

Plan of Correction: Corrected. The staff completed a new sworn disclosure statement while the licensing inspector was on site.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of five staff records on 12/10/19, the center did not obtain a central registry finding within 30 days of employment for one staff.

Evidence: 1) Staff #5, with an original hire date of 8/17/15, terminated employment on 8/6/18 and returned as a re-hire on 8/19/19. 2) The licensing inspector observed the central registry findings in the record for Staff #5 were dated 10/01/15. The staff had been terminated for more than 12 months and required a new central registry finding within 30 days of employment.

Plan of Correction: Staff completed central registry finding request form on 12/10/19 and will be mailed on 12/10/19. Verification will be sent to licensing as required.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of five staff records and interview on 12/10/19, the center did not ensure that two staff obtained a fingerprint based background check determination prior to employment.

Evidence: 1) The licensing inspector observed the fingerprint results in the record for Staff #4 were dated 9/19/18. 2) During interview, a member of management reported that Staff #4 terminated employment on 1/26/19, but was rehired on 8/19/19. New fingerprint results should have been obtained for Staff #4 prior to employment.

3) The licensing inspector observed the fingerprint results in the record for Staff #5 were dated 4/18/18. 4) During interview, a member of management reported that Staff #5 terminated employment on 8/6/18, but was rehired on 8/19/19. New fingerprint results should have been obtained for Staff #5 prior to employment.

Plan of Correction: Staff will request an appointment within next two days and will submit to fingerprints. Verification will be sent to licensing as required.
