An unannounced monitoring inspection was conducted beginning at 1:45 p.m. and ending at 4:00 p.m. with the director. There were 55 children in care ranging in age from 3 months to 5 years of age under the care and supervision of 7 staff. Children were observed during naptime. Infants were observed having playtime and sleeping. Center documents and 28 staff records were reviewed.

Standard #: 63.2(17)-1716-A-9 Description: Based on observation, it was determined that the center was not in compliance with all safe sleep guidelines recommended by the American Academy of Pediatrics. Evidence: An infant was observed sleeping in an infant swing with two blankets. A second infant was observed sleeping with a fleece blanket in the crib. Plan of Correction: Staff will be reminded of the safe sleep requirements.
Standard #: 63.2(17)-1716-B-3 Description: Based on review of documenation and interview with staff, it was determined that the center did not have current training to perform daily simple health screening and exclusion of sick children. Evidence: There were no staff with current Daily Health Observation (DHO) training. Plan of Correction: Training will be scheduled as soon as possible.
Standard #: 63.2(17)-1720.1-A Description: Based on review of documentation and interview with staff, it was determined that the center did not conduct background checks every five years as required. Evidence: Staff #5 most recent central registry background check was dated 8/11/2014. Staff #7 most recent sworn statement was dated 10/1/2013. Plan of Correction: Staff #7 completed an updated sworn statement during the inspection. Staff #5 had been given the central registry background check to complete and get notarized. Staff #5 returned the form during the inspection and it will be mailed as soon as possible.
