Comments:

An unannounced monitoring inspection was conducted from 1:00pm-2:00pm. Upon arrival there were 9 children in care, totaling 7 points and 2 caregivers. A sample of 5 children and 3 provider and household records were reviewed. Children were observed hand washing and participating in story time and nap. The areas of noncompliance are detailed in the Violation Notice and were discussed with the provider at the conclusion of this inspection.

Description:	Based on record review and interview it was determined the provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before a child may attend the family day home. Evidence: 1. The record for child #4(enrolled 8/19/19) did not contain documentation of immunization. 2. The provider confirmed the record for child #4 did not contain documentation of immunization.
Plan of Correction:	Provider will request immunization documentation from the parent at pick up.
