Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and video tour.

A monitoring inspection was conducted on 2/24/2022. The provider was contacted by phone to initiate the inspection. There were 12 children present with the provider and assistant provider. The Inspector reviewed 2 children?s records and 2 provider/household member records, along with any requested program records to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-800-800-A
Description: Based on record review and interview it was determined the provider did not annually review and update if needed the emergency preparedness plan.
Evidence:
The provider reported the emergency preparedness plan was last reviewed and updated two years ago.

Plan of Correction: Provider will document emergency preparedness plan review and update if needed.

Standard #: 8VAC20-800-60-B
Description: Based on record review it was determined the provider did not ensure each child's record contained documentation of the annual review of the child's emergency contact information.
Evidence:
1. The record for child #1(enrolled 8/20/18) did not contain documentation of an annual review and updated if needed of the child's emergency contact information.
2. The last documented date of review for child #1's emergency contact information was September 2020.

Plan of Correction: The provider will have parent review and update if applicable the emergency contacts in the child's record.

Standard #: 8VAC20-800-70-A
Description: Based on record review and interview it was determined the provider did not have documentation of parents being provided the provisions of the family day home's emergency preparedness and response plan before first day of attendance.
Evidence:
1. The record for child #1(first day of attendance 8/20/18) did not contain documentation of parent receiving the provisions of the family day home's emergency preparedness and response plan.
2. Provider confirmed that this documentation was not available during this inspection.

Plan of Correction: Provider will ensure provisions of the emergency preparedness plan are in the child's record.
