Comments:
An unannounced monitoring inspection was conducted on 5/7/2024 at 12pm. There were 8 children (16 points) directly supervised by 3 caregiver(s). The home, 5 caregiver records, 5 child's records, and household member's records, evacuation drills, emergency supplies, and policies were inspected. There were no children's medications on site today. Children were observed playing freely outside. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Description: Based on review, the provider did not ensure that they reviewed the emergency preparedness plan at least annually and updated the plan as needed. The provider shall document in writing each review and update to the emergency plan.
Evidence: reviewed the emergency preparedness plan and found that the last documented review was on 12/8/2022.

Description: Based on review, the provider did not ensure that each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a licensed physician, the physician's designee, or an official of a local health department.
