Comments:
An unannounced renewal inspection was conducted on 5/10/2024 from 1:30pm to 3:30pm. At the time of the inspection there were 31 children in care and 5 staff members. Children were observed during nap time. Records were reviewed for five children and four staff members. The inspector toured the facility and the playground. Medication, evacuation drills, emergency supplies and other required records and postings were also reviewed.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on a record review and interview, it was determined that the facility did not ensure that all children had documentation of a physical examination by or under the direction of a physician before the child?s attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #4 (enrolled on 11/27/23) did not have documentation of having received a physical.
2. The Director confirmed the physical information was missing.

Description: Based on review of children?s records and interviews, it was determined that the center did not ensure that each child?s record contains all required information.
Evidence:
1.The record for child #2 did not contain the parents work phone numbers.
2.The Director confirmed the phone numbers were not documented

Standard #: 8VAC20-780-270-A
Description: Repeat Violation
Based on observation and interviews, it was determined that the facility did not ensure all areas and equipment of the center shall be maintained in a safe and operable condition.
Evidence:
In the nap room, a power cord was stretched out and dangling from a television (TV) on a the end of a storage unit. The distance from the TV to the outlet on the wall was approximately 3 feet. This posed a hazard, as a child could pull the cord causing the TV to fall.
