Comments:
An unannounced non-mandated inspection took place on November 12, 2019, beginning at approximately 8:15 AM and ending at approximately 12:00 PM. Five staff records and five children's records were reviewed during the inspection. There were 27 children in care and 9 staff present. There were no medications onsite during this inspection. All areas that are used for care were observed, including vehicles and the playground. The inspector looked at first aid and emergency supplies, drill documentation, and required health and fire inspections. The children and staff were observed during the following activities: breakfast, media time, playing with dough, interacting with the class pet, and religious expression. There were discussions about the fire inspection, children's records, and future inspections. The violations were discussed at the conclusion of this inspection. Standard #: 22VAC40-185-260-B
Description: Based on observations and discussion with staff, the center failed to ensure that after the first license, annual approval from the health department shall be provided, or approvals of a plan of correction, for meeting requirements for: 1. Water supply; 2. Sewage disposal system; and 3. Food service, if applicable.

Evidence:

1. The Health Inspection for the center was dated 08/21/2018. The director stated that there was not newer inspection.

Plan of Correction: The director will reach out to the health department to obtain a new inspection and report.

Standard #: 22VAC40-185-290-3
Description: Based on observations and discussion with staff, the center failed to ensure that electrical outlets shall protective covers that are of a size that cannot be swallowed by children.

Evidence:

1. Power strips located in the two-year-old room and the baby room had power strips with approximately 4 outlets that were not covered with protective covers.
2. Office staff confirmed that the outlets were uncovered.

Plan of Correction: Outlets were covered with protective outlet covers during inspection.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center failed to ensure that when diapering, a nonabsorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or counter-top designated for changing.

Evidence:

The diaper pad located in "Butterfly 2" was torn, thereby potentially causing some areas of the diaper pad to be absorbent.

Plan of Correction: Staff will purchase a new pad today.
