We discussed the requirements for staff records. The risk assessment for a violation was provided. Also, we reviewed requirements for outdoor maintenance, locked areas for storage of hazardous substances and emergency response planning/training.

Thank you for your assistance during the unannounced monitoring inspection conducted from 10:30 AM to 4:30 PM to review supplemental health and safety requirements. Today, there were thirteen children in care during the first part of the morning and six to seven children, ages two through seven years, with two staff after the inspection began. Today was an unusual day of operation because a number of staff needed to be off and parents were notified to pick-up early or make other arrangements for their children since there were only two staff working. I viewed program activities, equipment, staffing, supervision, interactions with children, posted information, emergency supplies and drill records, emergency plan, injury records, updated injury prevention plan, diapering procedures, diaper ointment, sunscreen, agency inspection reports, five records for children, five staff records, staff qualifications, staff training, indoor and outdoor areas. The children enjoyed choices for indoor play, books and group discussions. They have been doing activities related to the weekly theme of summer. Children had opportunities for active play outside. Standard #: 22VAC40-185-70-A Description: Based on a review of five records for staff, and an interview with administrative staff, the center failed to obtain all required information for one staff member's record. Evidence: According to the record of staff member 3, the beginning date for hire as a lead teacher was May 1, 2019 and the education transcripts to demonstrate that staff member 3 qualified for the job position were not in the file. According to the resume, staff member 3 listed college attendance dates for "Education Studies, Liberal Arts". Plan of Correction: The director immediately contacted staff member 3 and asked for a copy of the college transcripts that included credits in the field of education. The education documentation will be viewed to make sure staff member 3 meets qualification requirements and placed with the record. The director will obtain documentation of the highest level of education completed for teachers who are hired.
