A Subsidy Health and Safety inspection was conducted from 10:30 AM to 4:30 PM to review supplemental health and safety requirements. Today, there were thirteen children in care during the first part of the morning and six to seven children, ages two through seven years, with two staff after the inspection began. I viewed record keeping, operational responsibilities, food service, children's information, caregiver training, emergency planning and drills, hazardous substance practices, behavioral guidance, hand washing, five records for children, and five staff records. No medication or food allergies at this time. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice.

Standard #: 22VAC40-665-540-A Description: Based on a review of five records for staff, and an interview with administrative staff, the subsidy vendor/center failed to obtain documentation of tuberculosis screenings completed within the last thirty days prior to initial employment of two staff. Evidence: The tuberculosis screening and statement for staff member 1 who began work with children on February 28, 2019 was obtained late on March 18, 2019. The tuberculosis screening statement for staff member 2 was completed on August 17, 2018 which was not within thirty days prior to the employment date of January 28, 2019. Plan of Correction: The director will make sure new employees obtain TB statements no earlier than thirty days prior to employment and before they begin work with children. A new employee in the hiring process has an appointment for a TB screening on June 19, 2019.
