Comments:

A Subsidy Health and Safety inspection was conducted from 10:52 AM to 3:40 PM to review supplemental health and safety requirements. Today, there were twenty-five children, toddler age through five years, with four staff. Three additional preschool age children arrived during the afternoon after attending other programs. Five school age children were receiving supervision after school this date. I viewed record keeping, operational responsibilities, food service, children's information, caregiver training, emergency planning and drills, hazardous substance practices, behavioral guidance, hand washing, five records for children, and five staff records. No medication or food allergies at this time. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Description:	Based on a review of five records for staff, and an interview with administrative staff, the vendor failed to ensure that all staff completed the Virginia Preservice Training within the first ninety days of employment. Evidence: Staff member 3 started work on May 1, 2019 and there was not a training certificate in the staff file. Administrative staff asked staff member 3 to access the certificate online and stated that there was message for resuming training at the segment where staff member last reached in the course.
Plan of Correction:	Staff member 3 will complete the VA Preservice Training and the director will obtain a copy for the staff file. In the future, the director will use a reminder system for checking on training progress by staff so that training is completed within the first ninety days of employment.
